GENERAL INSTRUCTIONS

If you received a Summary Notice of this Settlement (a Postcard or email), the Settlement Administrator identified you as a Settlement Class Member. You may submit a claim for settlement compensation, outlined below. Please refer to the Detailed Notice and the Agreement, both posted on this Settlement Website, for more information on submitting a Claim Form and for definitions of certain capitalized terms, respectively.

If you wish to receive compensation for Out-of-Pocket Losses or Lost Time spent dealing with the Data Breach, and/or an Alternative Cash Payment from this Settlement, you must submit the Claim Form below by May 18, 2026

You may submit a claim for the following benefits:

1); Reimbursement for Out-of-Pocket Losses: All Class Members who suffered an out-of-pocket expense fairly traceable to the Data Breach may submit a claim for a cash reimbursement. Out-of-Pocket losses are categorized as follows:

i. Ordinary Expenses. Defendant will pay valid claims for such ordinary expenses (up to $500.00). Ordinary expenses include (i) professional fees including attorneys’ fees, accountants’ fees, and fees for credit repair services; costs associated with freezing or unfreezing credit with any credit reporting agency; credit monitoring costs that were incurred between the date of the Data Breach and seven (7) days after Notice is sent to the Settlement Class; and miscellaneous expenses such as notary, fax, postage, copying, mileage, and long-distance telephone charges that were incurred between the date of the Data Breach and seven (7) days after Notice is sent to the Settlement Class. Ordinary losses include documented unreimbursed losses relating to fraud stemming from the Data Breach.

ii. Extraordinary Expenses. Next, Defendant will pay valid claims for extraordinary losses up to $5,000. Extraordinary expenses include proven monetary losses that meet the following conditions: (i) the loss is an actual, documented and unreimbursed monetary loss caused by (A) misuse of the Class Member’s personal information or (B) fraud or identity theft associated with the Settlement Class Member’s personal information; (ii) the loss was more likely than not caused by the Data Breach; (iii) the loss occurred between the date of the Data Breach and seven (7) days after Notice is sent to the Settlement Class; and (vi) the loss is not already covered by the Lost Time and Ordinary Expenses categories and the Settlement Class Member made reasonable efforts to avoid, or seek reimbursement for, the loss, including but not limited to exhaustion of all of the Settlement Class Member’s credit monitoring insurance and identity theft insurance.

2) Reimbursement for Lost Time: Class Members may make a claim for self-certified time spent related to the effects or potential effects of the Data Breach. Each Class Member may claim up to $90 of lost time ($22.50/hour, up to 4 hours) by writing a description of activities performed and attesting to the fact that they expended such time and describing how the time was spent.

3) Alternative Cash Payment: Settlement Class Members can elect to make a claim for a $50.00 Alternative Cash Payment in lieu of a claim for an Out-of-Pocket Loss or Lost Time. To receive this benefit, Settlement Class Members must submit a valid Claim Form, but no documentation is required to make a claim.

OPTION 1: COMPENSATION FOR OUT-OF-POCKET LOSSES
